PLMA 2009 Demand Response Award Nomination Form

The nominations must contain the following information.  If you are nominating more than one program, please use a separate nomination form for each.

  • Name of person and/or company being nominated.
  • Name, Address, phone number, and e-mail address of person submitting nomination
  • Program Name
  • Program Target Segment (e.g., residential, small commercial, large industrial, etc.)
  • Program Description
  • Why this program deserves an award (actual results are key)
    • What reduction was achieved as  % of supplier obligation?
    • Were rotating outages avoided?
    • Was the economic impact of capacity shortfalls reduced?
    • Was the cost of wholesale market purchases lowered?
    • Provide information on how the program meets long term needs, or maintains viability during non-crisis periods.
    • Describe what aspects of the program were particularly innovative.
  • Please attach any supporting materials (brochures, web site information, education materials, etc.).
