The nominations must contain the following information. If you are nominating more than one program, please use a separate nomination form for each.
1) Name of person and/or company being nominated.
2) Name, Address, phone number, and e-mail address of person submitting nomination
3) Program Name
4) Program Target Segment (e.g., residential, small commercial, large industrial, etc.)
5) Program Description
6) Why this program deserves an award (actual results are key)
a) What reduction was achieved as % of supplier obligation?
b) Were rotating outages avoided?
c) Was the economic impact of capacity shortfalls reduced?
d) Was the cost of wholesale market purchases lowered?
e)
Provide
information on how the program meets long term needs, or maintains viability
during non-crisis periods.
f)
Describe
what aspects of the program were particularly innovative.
